Rates: Initial visit $200, Return visit $150

Uninsured and Medicare patients may use our visit cost reduction sliding scale so that visits are affordable. This scale reduces the cost of your visit based on income, no check stubs needed, we use the honor system. A visit reduction form will need to be signed at time of income verification.
